Occupational Therapy Pre-Referral Form Permission Slip for Occupational Therapy

School based occupational therapy is designed to enhance the student’s ability to fully access and be successful in the learning environment. This might include working on fine motor skills such as handwriting, helping organize the working environment, and working with the student and teacher to modify or adapt learning materials to facilitate successful participation in the classroom. This is the third academic year that the St. Charles volunteer occupational therapy service has been available.

We are not affiliated with Monroe County Community School Corporation (MCCSC), but are simply parent volunteers offering our time. Your child may be entitled to additional services through MCCSC and any questions about this should be directed to your child’s teacher. We hope to be a resource for questions about occupational therapy and issues that might impede learning and function. There is no fee for this service, only a parental permission slip is required.
